Tijdens deze behandeling wordt er plasmedicatie gegeven via een infuus. De cardiologen in het ziekenhuis werken intensief samen met verpleegkundigen van thuiszorgorganisatie Zuidzorg om deze zorg mogelijk te maken.<\/strong><\/p>\n<p>Door hartfalen kan het voorkomen dat u teveel vocht gaat vasthouden. Om dit vocht te verliezen, helpen plastabletten niet altijd voldoende. De arts kan dan besluiten dat u plasmedicatie via het infuus nodig heeft om het teveel aan vocht kwijt te raken. Het ziekenhuis is hiervoor niet altijd de beste omgeving. Veel pati\u00ebnten slapen slechter, bewegen minder en eten slechter in het ziekenhuis dan thuis. Ook is de kans op een infectie of acute verwardheid in het ziekenhuis groter dan thuis. Als u in aanmerking komt voor een thuisbehandeling, zal uw arts dit met u bespreken.<\/p>\n<p>Een thuisbehandeling maakt het mogelijk om sneller te herstellen in uw eigen omgeving. U blijft in beweging, waardoor u uw spiermassa beter behoudt. Ook is het risico op infecties, vallen en verwardheid thuis een stuk kleiner dan in het ziekenhuis. De meeste mensen voelen zich thuis meer op hun gemak, wat uiteindelijk zorgt voor een sneller herstel.<\/p>\n<p>Tijdens de thuisbehandeling komt er dagelijks een verpleegkundige van de thuiszorg bij u thuis. De verpleegkundige beoordeelt hoe het met u gaat en neemt bloed af en sluit zo nodig nieuwe medicatie aan. Zelf meet u uw bloeddruk, hartslag en gewicht. Loopt u tegen problemen aan? Uw cardioloog heeft in overleg met u besloten dat u thuis behandeld wordt om verder te herstellen. We noemen dit Hartfalen@home, thuisbehandeling voor hartfalen met medicatie via het infuus. U hoeft deze infuuspomp zelf niet te bedienen. De pomp is aangesloten op het infuusnaaldje en dient continu een kleine hoeveelheid plasmedicatie toe. Bij ontslag krijgt u een \u2018thuisbehandeling koffer\u2019 met materiaal. U en de verpleegkundige gebruiken deze tijdens de thuisbehandeling. Zoals het meten van uw bloeddruk, hartslag en gewicht. Deze metingen noteert u in de thuismetenapp of in het hartfalen boekje. Heeft u klachten ervaren of sterk afwijkende metingen?
