Ziekenhuiszorg in je eigen woonkamer<\/h2>\n

Stel je voor: je hebt acuut hartfalen, maar in plaats van een week in het ziekenhuis te liggen, krijg je de behandeling gewoon thuis. Dat is precies wat Hartfalen@home mogelijk maakt. Het project is nu bekroond met de publieksprijs van de STZ Innovatiechallenge 2025.<\/strong><\/p>\n

Zorg zonder ziekenhuisbed<\/h3>\n

Hartfalen@home is een samenwerking tussen M\u00e1xima MC en ZuidZorg, waarbij pati\u00ebnten met acuut hartfalen thuis via een mobiele infuuspomp medicatie krijgen. Na de diagnose op de Eerste Hart Hulp of in een later stadium op de verpleegafdeling, kunnen ze dezelfde dag naar huis. Een gespecialiseerd thuiszorgteam van ZuidZorg begeleidt de pati\u00ebnt dagelijks. Een app ondersteunt met adviezen bij afwijkende metingen van bloeddruk, saturatie en gewicht. Zo blijft de pati\u00ebnt onder controle, zonder fysiek in het ziekenhuis te liggen.<\/p>\n

\u201cDankzij Hartfalen@home kunnen we ziekenhuiszorg bieden in de vertrouwde omgeving van de pati\u00ebnt \u2013 en d\u00e1t maakt het verschil,\u201d zegt Frank Gommans, cardioloog bij M\u00e1xima MC.<\/p>\n

Comfort, veiligheid en samenwerking<\/h3>\n

De resultaten zijn veelbelovend: pati\u00ebnten voelen zich comfortabeler en veiliger thuis, en waarderen de zorg met een rapportcijfer tussen de 8,0 en 8,4. \u201cWe zien dat deze vorm van hybride zorg echt werkt \u2013 voor de pati\u00ebnt \u00e9n voor de zorgprofessionals,\u201d aldus Lizette Dirks – van Beers verpleegkundige van het verpleegkundig technisch team bij ZuidZorg.<\/p>\n

In \u00e9\u00e9n jaar tijd zijn al 131 ligdagen naar huis verplaatst. Voor zorgprofessionals betekent dit meer ruimte in het ziekenhuis \u00e9n een intensieve samenwerking over domeinen heen: ziekenhuis, apotheker en thuiszorg werken dagelijks samen. \u201cWe verbeteren het zorgpad en de samenwerking door elke maand te evalueren en samen te werken in een projectgroep met alle betrokken partijen,\u201d aldus Mylene Frankfort, projectleider M\u00e1xima MC.<\/p>\n

Waarom dit belangrijk is<\/h3>\n

Voor pati\u00ebnten betekent Hartfalen@home minder risico op complicaties zoals infecties of valincidenten. Voor verpleegkundigen en artsen biedt het een nieuwe manier van samenwerken, waarbij scholing en vertrouwen centraal staan. En voor het zorgsysteem als geheel draagt het bij aan duurzame en toegankelijke zorg.<\/p>\n
