Wij leggen u graag uit hoe dat werkt.<\/p>\n<h2><strong>Hoe vraag ik het aan?<\/strong><\/h2>\n<p>U vraagt alle gegevens zoals hieronder beschreven op bij uw behandelend specialist in uw ziekenhuis. Volg hierbij de richtlijnen van het ziekenhuis waar u in behandeling bent. Wij vragen zelf geen gegevens op.<\/p>\n<h2>Benodigde gegevens<\/h2>\n<ul>\n<li>Verwijsbrief van uw huisarts met daarin:\n<ul>\n<li>1. of het gaat om een overname van behandeling of second opinion;<\/li>\n<li>2. bij second opinion: de vraagstelling van u en\/of uw arts (waar wilt u onze mening over?).<\/li>\n<\/ul>\n<\/li>\n<li>Medische gegevens zoals: alle correspondentie en operatieverslagen, uitslagen en medische beelden van radiologisch onderzoek en eventuele laboratorium uitslagen.<\/li>\n<li>Gegevens van uzelf: naam, adres, geboortedatum, telefoonnummer, emailadres en verzekeringsinformatie.<\/li>\n<li>Eventueel gegevens van een contactpersoon: naam, emailadres en telefoonnummer.<\/li>\n<\/ul>\n<p>Deze gegevens levert u zelf bij ons aan. Daarom plannen wij de afspraak in <strong>nadat <\/strong>uw medische gegevens zijn beoordeeld door onze specialist(en).<br \/>\nHierna kunnen wij u verder informeren over uw aanvraag.<\/p>\n<h2>Wanneer aanvragen?<\/h2>\n<p>Bij een second opinion zijn doorgaans de onderzoeken afgerond en het behandelplan bekend.
